Authorization, Consent, Waiver and Release

         I agree that Bay Area Tennis Society ("BATS") shall not be liable for any injuries or damages to my person or property that I might sustain or incur at or in connection with any BATS-sponsored events or activities.  Such injuries or damages shall include, without limitation, those that result from acts of active or passive negligence on the part of BATS or its officers, directors, members or agents. I agree, for myself and my legal representatives, to release, discharge and hold harmless BATS and its officers, members and agents from and against, and to waive all claims, demands and legal actions based upon any such injuries or damages. 

       

          I hereby authorize and consent to the use and publication by the Bay Area Tennis Society (BATS), for any lawful purpose, of my name and any photographs, image or likeness of me in connection with my participation or involvement in BATS’ events or activities. 

       

          I acknowledge that my participation in BATS’ events and activities is voluntary, and I hereby waive any separate or further consideration of any kind for this authorization and consent, including, without limitation, for any use or publication of any such photo, image or likeness in BATS’ digital and printed publications. 

       

           I also hereby release and hold harmless BATS and its officers, directors, members, agents, and assigns from and against any and all claims, causes of action and damages, known or unknown, that I or my heirs or representatives may have in connection with or arising out of any subject matter of this authorization and consent.
